Physical
Examinations
All student entering
high school for the first time must have a complete physical
examination, and we strongly recommend a dental exam. In order to comply with
the school code of Illinois, Section 27-8.1, it is imperative that the health
record be filled out and signed by a doctor. A complete immunization
history must be recorded on the State of Illinois Certificate of health
Examination form will all immunization dates, including those received as an
infant, recorded by day, month, and year. The health history section must
be completed and signed by a parent/guardian. For questions or more

Students who are not in compliance with health examination and immunization requirements will be contacted by the school nurse and will be excluded from attending classes until compliance is met.
Important Information for Athletes
All sophomores, juniors and seniors who plan on participating in a sport must have a participation form and sports physical on file with the Athletics Office before they will be allowed to tryout. Freshmen do not need a separate sports physical; their school physical is sufficient.
IHSA Steroid & Performance-Enhancing Supplement Testing
The Illinois High School Association (IHSA) randomly tests high school athletes for steroids and performance-enhancing supplements during state final competition. In order to be in compliance with the IHSA ruling, JCA must have a steroid testing consent form on file in the Athletic Office for any athlete participating in a varsity sport.
